29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Location: Worksite On or about June 3, 2025, an employee was exposed to a fall hazard while using a portable folding ladder where the employer did not instruct each employee on the manufacture's safety instructions regarding it's proper use. Actual Incident: On June 3, 2025, an employee was seriously injured and hospitalize after the ladder in use slipped out causing the employee to fall to the stone patio below.
