29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: (a) On or about June 3, 2025, at the jobsite located at 2060 Arthur Ave, Racine, WI 53405, the employer had not ensured the attachment point of the body harness was located in the center of the wearer's back by clipping it to the D-Ring on the harness, while performing roofing work on an existing residential home. This exposed employees to a approximately 10 feet fall.
