29 CFR  1926.20(b)(2):  Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:  On June 4, 2025, at a worksite located at 656 N. Main Street in Oshkosh, WI; the employer did not provide for frequent and regular inspections of the job site to be made by competent persons designated by the employer. Subcontract employees of the employer were exposed to fall hazards and eye hazards from flying particulate. Subcontract employees were not wearing personal fall harnesses properly, were connecting personal fall protection systems to hinged roof anchors which were not properly fastened, were connecting two persons' fall protection systems to a single hinged roof anchor, and were wearing sunglasses which were not rated for impact protection.

29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On June 4, 2025, at a worksite located at 656 N. Main Street in Oshkosh, WI; the employer did not ensure that subcontract employees operating pneumatic nail guns used appropriate eye or face protection when the potential for eye injuries from flying particles were present. An employee was wearing sunglasses which were not rated for impact protection.

29 CFR  1926.502(d)(15)(i):  Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two:  At a worksite located at 656 N. Main Street in Oshkosh, WI; anchorages used for attachment of personal fall arrest equipment were not installed and used as designed for part of a complete personal fall arrest system when employees were engaged in the installation of a roofing materials on a new construction multi-family residential building with eve edges 19 feet above the lower level:  a) On June 4, 2025, there were two instance of multiple subcontract employees bring connected to a single hinged roof anchor which would not be capable of arresting the fall of both employees.   b)  On June 4, 2025, multiple hinged roof anchors, to which subcontract employees' personal fall protection systems were connected, were not installed with the required number of fasteners according to manufacturer's requirements.

The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head:  On June 4, 2025, at a worksite located at 656 N. Main Street in Oshkosh, WI: subcontract employees engaged in the installation of a roofing materials on a new construction multi-family residential building with eve edges 19 feet above the lower level, did not have their body harness attachment points located at the center of the wearers' back near shoulder level. Two employees were observed with their attachment points near their middle/lower back.
