29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles: On or about June 2, 2025, at 6359 Stumph Road, Parma Heights, OH 44130, the employer failed to ensure employees using a Pierce pneumatic nail gun to install shingles on a residential home were utilizing appropriate eye or face protection when exposed to flying particles. Lux Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1668480, citation number 01, item number 001 and was affirmed as a final order on June 16, 2023, with respect to a workplace located at 21000 Lakeshore Blvd, Euclid, OH 44123.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about June 2, 2025, at 6359 Stumph Road, Parma Heights, Oh 44130, the employer failed to ensure employees performing weatherproofing on a 4:12 pitch roof were protected against a fall of approximately 10 feet in that no personal fall arrest systems/devices were utilized. Lux Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1668480, citation number 01, item number 002 and was affirmed as a final order on June 16, 2023, with respect to a workplace located at 2100 Lakeshore Blvd, Euclid OH 44123.
