29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a) On June 02, 2025, and at times prior, the employer did not ensure that modifications and/or additions were performed with manufacturers prior written approval as required.  The CAT TH560B,  340405 Diesel forklift, model number TH350B, was equipped with 42-inch tines that had been modified. The left tine had aftermarket holes located approximately 4 inches from the tip. The employer had not received prior written permission from the forklift manufacturer to modify the attachment. The modification to the forks potentially affects the capacity and safe operation exposing employees to struck-by hazards.
