29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: 869 Quaker Highway, Uxbridge, MA: On or about April 1, 2025, and times prior, sub-contractors and employees operated a Toyota electric fork truck, Model 8FBCHU25, serial # 64998 that was equipped with a Cascade Corporations Carton Clamp, Model Number 14J-CCS-2A-5074. The truck data plate was not marked to identify the carton clamp and did not show updated truck weight and load capacities.
