The employer did not initiate or maintain an accident prevention program to address the associated hazards that employees are exposed to during masonry activities, such as but not limited to fall hazards.

29 CFR 1926.453(b)(2)(v): 29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Jobsite located at 163 N 3rd St, Philadelphia, PA 19106 - On or about June 5, 2025, the employer did not ensure employees working in an aerial lift wore a personal fall arrest or travel restraint system. Employees were preforming masonry work on the exterior of the building in an aerial lift without a lanyard attached to the boom or basket, thereby exposing employees to falls of approximately 30 feet.
