29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury a) Crusher Area, East side of site: On or about May 10, 2025, and at times prior, the employer exposed employees to struck-by hazards from dislodged material, in that employees were not instructed in the recognition and avoidance of unsafe conditions while performing blockage removal in a TJ-2745 mobile jaw crusher. Specifically, an employee used a torch hose to clear a jam in the crusher without supervision or training on safe clearing methods resulting in a piece of metal that became dislodged and struck the employee in the face, causing serious injury.
