29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone: a) On or about June 5, 2025, at the residential property located at 5700 Old Ocean Blvd, Unit # 4P, Ocean Ridge, FL, 33435, employees were exposed to a 17-feet fall hazard while installing a new air-conditioning unit on a low-slope roof without any conventional means of a fall protection system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder: a) On or about June 5, 2025, at the residential property located at 5700 Old Ocean Blvd, Unit # 4P, Ocean Ridge, FL, 33435, employees were exposed to an approximate 17-foot fall hazard while performing roof wood decking using a ladder that was not extended to at least 3 feet (.9m) above the upper landing surface and not secured at its top to a ridgid support.
