29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:   (a) South Press Room : On June 5, 2025, the employer did not ensure all employees that serviced equipment were trained as "authorized" employees. Employees performing die setting tasks on mechanical power presses such as the Schuler Presses #64, #65 and #66 were not provided with the knowledge and skills needed for the safe application, usage, and removal of energy control.  No abatement certification or documentation is required for this item.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (a)South Press Room :  On June 4, 2025, employees were exposed to machine hazards associated with moving parts when troubleshooting mechanical power presses such as the Schuler Presses #64. The employer did not implement energy control application steps such as lockout tagout when performing die setting [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving dissipation machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.  (b) South Press Room: On June 5, 2025, employees were exposed to machine hazards associated with moving parts when conducting die setting on mechanical power presses such as the Schuler Presses #64, #65 and #66 . The employer did not implement energy control application steps such as lockout tagout when performing die setting [per the 1910.147(d)(2) requirements]. 29 CFR  1910.217(c)(3)(iii)(f): Mechanical power press(es) using presence sensing point of operation devices were not provided with guards to protect all areas of entry to the point of operation not protected by the presence sensing device(s)  (a) South Press Room: On or about June 4, 2025, the employer did not ensure mechanical power presses were guarded or had installed presence sensing devices which offered protection to all areas of entry to equipment such as but not limited to the mechanical power press Schuler #64.   No abatement certification or documentation is required for this item.
