On or about May 11, 2025, on the second level of a residential structure, an employee was not in use of fall protection while performing roofing work activities at a height greater than six (6) feet.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. On or about May 11, 2025 and at times prior, at a residential roofing site, employees performing roofing work activities were not instructed in the recognition and avoidance of unsafe conditions while working at heights.
