29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping or has guardrails which block employee access to the cantilevered end. a. On or about June 3, 2025, at the above addressed jobsite, an employee was exposed to falls while performing siding operations from a ladder jack scaffold, approximately eleven (11) feet above the ground, that had the working plank that exceeded eighteen (18) inches past the support beam on the east side.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a. On or about June 3, 2025, at the above addressed jobsite, an employee conducting siding operations from a ladder jack scaffold, approximately eleven (11) above the ground, was not provided with an access ladder, thereby exposing the employee to falls.
