29 CFR 1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about June 5, 2025, during the installation of roofing material at a project located at 23414 Cedar RD, Beachwood Ohio 44122, the employer did not ensure that the ladder rails on the portable ladder used by employee(s) to gain access/egress to the roof were extended 3 feet above the landing surface.

29 CFR 1926.501(b)(11): Steep Roofs: Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems: On or about June 5, 2025, during a construction roofing project on a residential structure located at 23414 Cedar RD, Beachwood Ohio 44122. The employer did not ensure employees working on a roof were protected from fall heights of greater than 18.3 feet, by utilizing a guardrail system with toe boards, safety net systems, or personal fall arrest systems. Ken's Parkhill Roofing LLC was previously cited for a violation of this occupational safety and health standard (1926.501(b)(11) Steep Roofs), which was contained in OSHA inspection number 1492424, citation number 01, item number 001 and was affirmed as a final order on 10/14/2020, with respect to a workplace located at 4759 Mayfield Road, South Euclid, Ohio, 44121.
