29 CFR 1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about June 6, 2025, at a job site located at 6901 ST Clair Ave, Cleveland, Ohio 44103; during the installation of roofing material on a multi-family residential structure, the employer did not ensure all employees using an aerial lift were protected from fall hazards approximately 23.7 feet by a fall protection device.
