29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. 1) J&M Precision Die Casting on or about June 6, 2025 at the location of 1329 Taylor St. Elyria, Ohio 44035: The employer did not establish an energy control program consisting of written machine specific procedures, training, and periodic inspections. Without these program components, employees tasked with performing lock out covered activities such as die changes, service and maintenance on the zinc and aluminum casting machines, were exposed to crushing and burn hazards.
