29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. (a) Clendenin Interchange Bridge: On or about June 5, 2025, an employee was performing concrete cutting and shaping tasks using a DeWalt handheld 4.5 inch grinder and was not wearing impact rated safety glasses with side shields, exposing the employee to eye injuries from flying concrete chips and debris. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section. (a) Clendenin Interchange Bridge: On or about June 5, 2025, employees used a DeWalt handheld grinder to cut and reshape concrete barriers without the manufacturer supplied guard being installed, exposing the employees to struck-by hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

(a) Clendenin Interchange Bridge: On or about June 5, 2025, employees were operating and working in close proximity to a handheld grinder used to grind and cut concrete without required engineering controls, exposing them to respirable crystalline silica. No abatement certification or documentation required.
