29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) On or about 6/4/25, at the mechanical room area of the site, Amherst, NY. Drain hole, of 6-inch diameter with a 3-inch PVC pipe, was not protected from tripping in or stepping into in active work area. PVC pipe had 5-inch "reveal" above ground. NO ABATEMENT CERTIFICATION REQUIRED
