29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. a.) On or about May 29, 2025, the employer failed to provide frequent and regular job-site inspections and in turn exposed an employee to a fall hazard from a fabricated triangle bracket and plank type scaffold, with no fall protection system in place, used to install vapor barrier on the second level exterior wall of a newly constructed residential house. As a result employees were exposed to 18' high fall hazards.

29 CFR 1926.451(g)(1):Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. a) On or about?May?29,?2025, the employer failed to ensure employees were protected from falling while erecting a fabricated triangle?bracket plank?type scaffold to install a vapor barrier on the second?story exterior wall of a newly constructed residence. As a result, employees working approximately 18?feet above the lower level were exposed to fall hazards.
