29 CFR  1910.28(b)(1)(i):Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):   On or about June 6, 2025, at the work located at 1818 N. Main ST, Nile Ohio 4446; the employer did not ensure that employees accessing the HVAC and Oven units on the Age Ovens that are located on the roof of the oven with unprotected sides and edges were protected from falls greater than 4 feet above the ground. The walking/working surface was 8.11 feet above ground.

A chain sling was observed attached to the overhead underhung crane and steel die with no identification tag.

29 CFR  1910.184(e)(3)(i): A thorough periodic inspection of alloy steel chain slings was not made on a regular basis and/or were done at intervals greater than once every 12 months:   On or about June 6, 2025, and at times before, the employer failed to ensure employees were protected from crushing hazards by ensuring thorough inspections were performed on the alloy chain slings that were being utilized to load/unload and move aluminum billets and die and other material around the facility.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.	On or about June 6, 2025, the employer did not ensure operator(s) of the #3 Aluminum Extrusion Press (WEAN United 1675), at the work location 1818 N. Main ST, Niles Ohio 4446 were protected by one or more methods of machine guarding from the moving and rotating parts of the powered conveyer and material puller system. The barrier guarding has breaks/gaps in multiple places allowing the operators/employees/crew to access/egress the conveyer systems (puller area and oven/ram loader area) while in operation. The operator/employees/crew had access to enter the area of operation by passing through and or walking around the barrier guards that partially separate the oven and press. This area is partially guarded with a barrier guard that measured 10 inches wide by 61 inches tall leaving a gap of approximately 11 inches wide between the press and barrier guard.   b.	On or about June 6, 2025, the employer did not ensure that operator(s) of the #1 Aluminum Extrusion Press (Youngstown Press) were protected by one or more methods of machine guarding from moving and rotating parts of the powered conveyor and material puller system.  The operator/employees/crew had access to enter the area of operation by passing through and or walking around the barrier guards that partially separate operator station and power conveyer and puller systems. There is a gap in the perimeter barrier that measured 61 inches wide that allows/allowed the operators access to the powered conveyer and puller system area exposing the employees to struck by and or caught in hazards while the employees access that area to move, handle, and or manipulate the extruded aluminum material  c.	On or about June 6, 2025, the employer did not ensure operator(s) of the  #2 Aluminum Extrusion Press (Ferrell 2000T 8-inch press), at the work location 1818 N. Main ST, Niles Ohio 4446 were protected by one or more methods of machine guarding from the moving and rotating parts of the powered conveyer and material puller system. The barrier guarding has breaks/gaps in multiple places allowing the operators/employees/crew to access/egress the conveyer systems (puller area and oven/ram loader area) while in operation.  Press #2 has a perimeter barrier guard (fencing), There is a gap in the perimeter barrier that measured 23 inches wide that allows/allowed the operators access to the powered conveyer and puller system area exposing the employees to struck by and or caught in hazards while the employees access that area to move, handle, and or manipulate the extruded aluminum material.  d.	On or about June 6, 2025, the employer did not ensure operator(s) of the #4 Aluminum Extrusion Press (Youngstown 2200T), at the work location 1818 N. Main ST, Niles Ohio 4446 were protected by one or more methods of machine guarding from the moving and rotating parts of the powered conveyer and material puller system. The #4 press has a damaged section (separation of multiple sections of fence) of the perimeter fence that maintenance employees utilized as access/egress at the North end of the powered conveyer and puller system to gain access/egress to the puller system and saw/cut bot (to add oil and maintain the system multiple time a shift) exposing the employees to struck by and or caught in hazards while the employees access that area to move, handle, and or manipulate the extruded aluminum material.

29 CFR  1910.212(a)(3)(iii):Special hand tools for placing and removing material(s) were not provided to permit easy handling of material without the operator placing a hand in the danger zone:  On or about June 6, 2025, during the operation of the five Aluminum Extrusion Presses, Press #1 (Youngstown Press), Press #2 (Ferrell 2000T 8-inch press), Press #3 (WEAN United 1675), Press #4 (Youngstown 2200T), and Press #5 (Field Plat 2700T) at the work location 1818 N. Main ST, Niles Ohio 4446 the employer did not ensure in house handmade tools such as tongs, 3 foot lengths of rebar with hooks bent on the end, and leverage bars used to manually manipulate billet aluminum and extruded aluminum prevented operators from placing their hands, arms, and body in the unguarded danger zone of the saw/cut box, powered conveyer, and puller systems and points of operation, and moving varying sizes of extruded aluminum product.  Employees utilize a short (less than 12 inches) bar to align material in the saw/cut box of the #3 Press that require the employee to lean into the saw/cut box up to 42 inches (waste deep) to reach the misaligned material exposing the employees to struck/caught by hazards.
