29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. On or about May 30, 2025, and at times prior thereto, at the Mustang Park construction site, employees who worked from a walking/working surface were exposed to a fall and tip-over hazard due to an unprotected floor hole.
