29 CFR 1910.147(d)(3):All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about June 4, 2025, and at times prior, at the establishment located at 531 Northwest Tyler Street, Topeka, KS; employees were exposed to struck-by and crushing hazards. Employees were removing set up concrete with a jack hammer inside a mixer drum without having energy isolating devices installed prior to beginning work.

Employees were exposed to the hazards associated with permit-required confined space hazards, such as but not limited to being struck against a rotating barrel.
