29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) 101 Mundis Race Rd YORK, PA 17406 - Maintenance employees were exposed to uncontrolled hazardous energy while conducting preventative maintenance and repair procedures on facility machinery to include but not limited to the Peddimax 88, "Iron Worker," # 4423551594006 due to the employer not establishing machine specific lockout / tagout procedures, on or about June 10, 2025. (b) 101 Mundis Race Rd YORK, PA 17406 - Maintenance employees were exposed to uncontrolled hazardous energy while conducting preventative maintenance and repair procedures on facility machinery to include but not limited to the Vertical bandsaw identified as V-18A due to the employer not establishing machine specific lockout / tagout procedures, on or about June 10, 2025. (c) 101 Mundis Race Rd YORK, PA 17406 - machine operators were exposed to uncontrolled hazardous energy while preforming machine set-up for the Vertical bandsaw identified as V-18A, due to the employer not establishing machine specific lockout / tagout procedures, on or about June 10, 2025. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
