29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Profiler Line 1: On or about June 10, 2025, employees working on Profiler Line 1 were exposed to amputation hazards from in-going nip points on the conveyor belt. b) Profiler Line 2: On or about June 10, 2025, employees working on Profiler Line 2 were exposed to amputation hazards from in-going nip points caused by rollers on the conveyor belt. c) Grinder: On or about June 24, 2025, employees working in the Grinder Room were exposed to amputation hazards from the auger that sifted waste product. American Flooring, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1782084, citation number 1, item number 2 and was affirmed as a final order on February 12, 2025, with respect to a workplace located at 160-180 Independence Drive, Pittston, PA 18640.
