29 CFR 1904.39(a)(2): Employer failed to report within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. (a) Inter Mountain Cable Inc, located at 20 Laynesville Road, Harold KY 41635: On or about June 5, 2025, an employee received injuries requiring inpatient hospitalization after a fall from a ladder. The employer failed to report to OSHA within twenty-four (24) hours of the in-patient hospitalization. No abatement certification or documentation required.
