29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge: (a) 215 Oberlin Road, Middletown, PA; overhang roof of the maintenance garage - On or about June 10, 2025, multiple employees who were working on the roof were using a warning line as a means of fall prevention which was placed at the roof edge instead of 6 feet from the roof edge. The employees were exposed to a fall hazard of approximately 25 feet to a lower level. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines: (a) 215 Oberlin Road, Middletown, PA; overhang roof of the maintenance garage - On or about June 10, 2025, multiple employees who were working on the roof were using a warning line which did not have an access path from the 32-foot extension ladder to inside the area marked out by the warning line. The employees were exposed to a fall hazard of approximately 25 feet to a lower level. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
