On a commercial building, at a temporally worksite located at 8117 Atlantic Boulevard in Jacksonville FL 32211: On or about June 11, 2025, the employer exposed employees to head injuries, in that employees conducting work at the roof level were throwing roofing material down to a dumpster while employees not utilizing protective helmets (hard hats  were working on clean up on the ground level .
