29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  On or about June 11, 2025, four employees were observed working on a residential roof at heights greater than 6 feet without being  protected by guardrail systems, safety net system, or personal fall arrest system, nor were they provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  The D & S Home Improvement & Remodeling LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1767998, citation number 1, item number 1 and was affirmed as a final order on November 5, 2024, with respect to a workplace located at 10325 S Willow Creek Dr Oak Creek, WI 53154.  The Samuel Olguin DBA D &S Home Improvement LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1757167, citation number 1, item number 1 and was affirmed as a final order on October 23, 2024, with respect to a workplace located at 2020 & 2022 Jubilee Ln Dunlap, IL 61525.  The D & S Home Improvements LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1605966, citation number 1, item number 1 and was affirmed as a final order on July 5, 2022, with respect to a workplace located at N2734 Baum Lane Appleton, Wisconsin 54913. 29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 11, 2025, the ladders on site for employees to access the roof did not extend at least 3 feet above the upper landing surface exposing employees to potential fall hazards.
