29 CFR 1910.178(l)(4)(iii): Refresher training and evaluation. An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. (a) On or about June 12, 2025, the employer failed to ensure that each powered industrial truck (PIT) operator's performance was evaluated at least once every three years. As a result, employees continued operating PITs without the required periodic evaluation to verify that they retained the knowledge and skills necessary for safe operation.
