29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Single-family home roof system, various locations; On or about June 10, 2025, in regard to employees who were not protected from falls while engaged in roofing replacement activities from a 6:12 pitch roof at heights ranging between approximately 13-feet, 4-inches to 21-feet, 6-inches above ground level, the employer did not implement a training program to enable each employee to effectively recognize and mitigate fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Single-family home roof system, southwest edge; On or about June 10, 2025, a Werner brand multi-position aluminum ladder (model MT 221AAL), configured as an extension ladder, was used by employees to access and egress the rooftop and the upper side rails did not extend at least 3-feet above the landing surface.
