On or about June 11, 2025, and at times prior, located in the thermal spray department, initial personal breathing zone air monitoring for chromium (VI) was not conducted for employees who thermal sprayed airline parts using high velocity oxygen fuel (HVOF) that may produce chromium (VI). b.) On or about June 11, 2025, and at times prior, located in the grind shop, initial personal breathing zone air monitoring for chromium (VI) was not conducted for employees who grind on airline parts containing chromium (VI).
