29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about June 11, 2025, at the above addressed jobsite, an employee using a pneumatic nail gun was not wearing eye protection, thereby exposing the employee to hazards associated with eye injuries.

On or about June 11, 2025, at the above addressed jobsite, employees performing roofing activities, ten (10) feet five (5) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.  b.  On or about June 11, 2025, at the above addressed jobsite, an employee performing roofing activities, nineteen (19) feet one (1) inch above ground, was not protected from falls, thereby exposing the employee to fall hazards.

On or about June 11, 2025, at the above addressed jobsite, employees performing residential roofing operations had been provided with fall protection training; however, the employer failed to verify the training by preparing a written certification of training record.
