1926.454(c) The employer had reason to believe that an employee lacked the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds, and the employer did not retrain such employee so that the requisite proficiency was regained:   (a) 215 Oberlin Road, Middletown, PA; steel erection site - On or about June 10, 2025, an employee stood on and climbed on the guardrails of a MEC model 2632se scissor lift to access and egress the horizontal steel I-beam, instead of positioning the machine so that direct access could be made within 24 inches above or below the point of access. The employee was exposed to a fall hazard of approximately 15 feet to concrete.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
