29 CFR  1926.552(a)(1):The employer did not comply with manufacturer's specifications and limitations applicable to the operation of all hoists and elevators.  a) Residential Site: On or about 6/6/2025, and at times prior, the employer exposed employees to fall and struck-by hazards in that employees used the material hoist designed for transporting roof shingles as access to a 10:12 pitch roof at heights up to 24 feet to ground level for roofing activities.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Residential Site: On or about 6/6/25, and at times prior, the employer exposed employees to fall hazards, in that employees were using a portable ladder to access the roof that was only extended approximately two feet from the upper landing to perform roofing activities on a 10:12 pitch roof.
