29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. On or about June 6, 2025, employees were engaged in powered industrial truck operations at 8551 Whitmore Circle Omaha, NE 68122, were exposed to struck-by hazards in that the employees were using in house fabricated securing pin made of a piece of steel welded to a section of rebar as means of securing the tow strap from one forklift to another forklift. The result was a catastrophic energy release when the connection point failed, and the strap flew back striking an employee in the face.
