29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about June 13, 2025, at a residential new home build job site, located at 470 Regency Park Dr., Tallmadge OH 44278, the employer failed to ensure employees, who were working on a residential roof of approximately 25 feet, were protected from a fall from height hazards in that employees were not wearing a personal fall protection harness nor were there any other fall protections in place.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: On or about June 13, 2025, at a residential new home build job site, located at 470 Regency Park Dr., Tallmadge OH 44278, the employer failed to train employees in hazard recognition the employer failed to train employees in the recognition of potential fall hazards and effective means of prevention and protection from such. Employees are exposed to a fall from height hazard of over 25 feet.
