29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. a) Menasha Packaging Company, LLC Philadelphia Display Group located at 601 E. Erie Street, Philadelphia, PA - On or about June 9, 2025, the employer did not ensure lockout/tagout procedures were utilized while performing cleaning maintenance on the Bobst Asitrade laminator 8950 machine. An employee performed cleaning maintenance on the machine without de-energizing the system, thereby exposing employee to caught-in between hazards.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Menasha Packaging Company, LLC Philadelphia Display Group located at 601 E. Erie Street, Philadelphia, PA - On or about June 9, 2025, the employer did not ensure that the rollers carrying materials for laminating on the Bobst Laminating machine were properly guarded while the machine was in use, exposing employees to amputation and crush hazards.
