29 CFR  1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  On or about June 13, 2025, at a multi unit residential apartment job site, located at 1514 Hampton Knoll Drive, Akron OH 44313,  the employer failed to conduct site safety inspections. These inspections aid in determining what potential hazards may exist and how to protect employees from such by implementing the use of various personal protective equipment, such as but not limited to the use of a fall protection systems and/or personal fall arrest harnesses.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures:     On or about June 13, 2025, at a multi unit residential apartment job site, located at 1514 Hampton Knoll Drive, Akron OH 44313, the employer failed to ensure employees, who were working on a steep roof at heights of over 35 feet, were protected from a fall from height hazards in that employees were not wearing a personal fall protection harness nor were there any other fall protections in place.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about June 13, 2025, at a multi unit residential apartment job site, located at 1514 Hampton Knoll Drive, Akron OH 44313, the employer failed to train employees in hazard recognition the employer failed to train employees in the recognition of potential fall hazards and effective means of prevention and protection from such.  Employees are exposed to a fall from height hazard of over 35 feet.

29 CFR  1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:  On or about June 13, 2025, at a multi unit residential apartment job site, located at 1514 Hampton Knoll Drive, Akron OH 44313, the employer failed to ensure that employees using portable extension ladders where trained in how to properly place and use the extension ladders, which were used to access a steep roof (35+ feet).  The ladders were not extended at least 3 feet/rungs above the upper landing surface.
