29 CFR  1926.1053(b)(1):When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access.  a) Jobsite: 735 Darien St. Philadelphia, PA - On or about June 13, 2025, the employer did not ensure the side rails of a portable ladder used for access to an upper landing surface extended at least 3 feet above the upper landing surface being accessed. Employees utilized a portable ladder to access a third story roof without having side rails extended at least 3 feet above the landing surface, thereby exposing employees to fall hazards.
