29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: A) By Reactor Tank 2: On or about June 4, 2025, the employer exposed employees to struck-by and thermal hazards, in that personal protective equipment such as a heat resistant jacket and a face shield were not worn while employees cleaned the line after transferring Suremix in the reactors.
