29 CFR 1926.502(d)(6)(i): A snap hook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snap hook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snap hook such that unintentional disengagement could occur by the connected object being able to depress the snap hook keeper and release itself.   a) New single-family structure located on Lot #43C at 159 Applewood Way Homer, GA 30547, Roof: On or about 06/12/2025, employees were exposed to fall hazards of approximately 20 feet, 10 inches to the lower level, in that an employee's personal fall arrest system was anchored by tying the lifeline in a knot around a roof truss, which is not an acceptable or secure anchorage method under the manufacturer's recommendations.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:   a) New single-family structure located on Lot #43C at 159 Applewood Way Homer, GA 30547, Roof: On or about 06/12/2025, the employer exposed employees to fall hazards of 20 feet, 10 inches to the lower level, in that the employees attachment point was not located in the center of the wearer's back while engaged in roofing activities on an 8/12 pitched roof.

29 CFR 1926.102(a)(1) :Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) New single-family structure located on Lot #43C at 159 Applewood Way Homer, GA 30547, Roof: On or about 06/12/2025, the employer exposed employees to struck-by hazards while operating pneumatic nail guns, in that protective eyewear was not provided while engaged in roofing activities.   M&S Roofing Construction LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number #1738537, citation number 1, item number 1 and was affirmed as a final order on 05/30/2024, with respect to a workplace located at Lot 151-155, Cambridge at Town Center Subdivision, Hoschton GA 30548.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     a) New single-family structure located on Lot #43C at 159 Applewood Way Homer, GA 30547, Roof: On or about 06/12/2025, the employer exposed employees to fall hazards of 20 feet,10 inches to the lower ground level, in  that complete fall protection systems were not provided or used while employees were engaged in roofing activities.  M&S Roofing Construction LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number #1738537, citation number 1, item number 2 and was affirmed as a final order on 05/30/2024, with respect to a workplace located at Lot 151-155, Cambridge at Town Center Subdivision, Hoschton GA 30548.

29 CFR 1926.552(a)(4): Hoisting ropes were not installed in accordance with the wire rope manufacturers' recommendations.   a) New single-family structure located on Lot #43C at 159 Applewood Way Homer, GA 30547, Roof: On or about 06/12/2025, the employer had replaced the manufacturer-required wire rope with nylon rope, which is not recommended for safe operation of the material hoist.
