29 CFR 1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) New residential construction located on  Lot 44  Maplebrook Lane, Hoschton, GA, 30548, Scaffold: On or about 06/16/2026, the employer exposed employees to struck-by hazards while operating pneumatic nail guns, in that eye protection was not provided as employees engaged in siding activities were operating these tools at 95 PSI.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a) New residential construction located on Lot 44 Maplebrook Lane, Hoschton, GA, 30548, Scaffold: On or about 06/16/2026, the employer exposed employees to fall hazards of 20-feet, 3-inches to the lower level, in that a fall protection system was not provided to employees engaged in siding activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) New residential construction located on Lot 44 Maplebrook Lane, Hoschton, GA, 30548, Front side of structure: On or about 06/16/2026; the employer exposed employees to fall hazards of 20-feet to the lower level, in that the ladder used was not extended at least 3-feet to allow for safe access and egress from the walking/working surface.

29 CFR  1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.   a) New residential construction located on Lot 44 Maplebrook Lane, Hoschton, GA 30548, Front side of structure: On or about June 16, 2026, the employer did not remove from service a Werner 20-foot extension ladder that was missing the right foot. Employees used the defective ladder, exposing them to fall hazards up to 20 feet to the lower level.
