Missouri Rd, Clearwater, FL 33756, the employer exposed employees to fall and crushed-by hazards, as the Skyjack SJIII 3219 scissor lift, Serial # 22052167 produced a speed of approximately 2.6 mph, in excess of 0.3 mps (0.68 mph), while being ridden and operated at elevations by employees.
