29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): a) Work site - On or about June 16, 2025 and at times prior thereto, employees were working from an approximately 5-7 foot deep excavation that was not protected from cave-ins having near vertical walls and no support, shield, or protective systems in place.
