29 CFR 1926.501(b)(10): Employee(s) engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8m) or more above lower levels were not protected from falling by a guardrail system, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) On or about 05/19/2025, at a project located at 460 Village II Drive, Hilton New York; an employee was installing roofing membrane on the roof of a residential apartment building. The roof was flat and the employee was working at the roof's edge without fall protections. The employee fell from a 19 feet roof to the ground below and sustained serious back and ribs injuries. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) On or about 05/19/2025 at a work site located 460 Village II Drive, Hilton, NY; an employee was replacing a roof membrane on an apartment building when he fell approximately 19 feet and was admitted to the hospital for approximately 3 days. The hospitalization was not reported by the employer. NO ABATEMENT CERTIFICATION REQUIRED
