29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Job site, 50 26th Street, Pittsburgh, PA: On or about May 23, 2025 during shoring assembly activities, the employer did not ensure employees were properly trained in scaffold safety practices. Employees were permitted to tie off to free standing shoring systems during erection, which did not stop them from reaching the ground if they fell and could cause the shoring system to come down onto them if they fell.
