29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place: At the worksite located at 1490 Southern Rd, Kansas City Missouri 64120 employees were exposed to struck by and crushed by hazards while engaged in material handling activities. The employer did not ensure that powered industrial trucks that employees were operating in the workplace had all labels in place on the operator control levers. The powered industrial truck with missing labels on the control levers is the sit-down Raymond forklift #4460C40.
