29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about 6/17/2024, at 118 Cayuga Street, Syracuse NY, 13204: A sub-contractors employee used an extension ladder to get off the roof and the ladder was not extended 3 feet above the edge of the roof exposing the employee to a fall hazard that measured 126 inches from the eaves to the ground.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) On or about 6/17/2024, at 118 Cayuga Street, Syracuse NY, 13204: A sub-contractor employee was installing roofing material on the low sloped porch roof, estimated to be a 2:12 pitch, without fall protection and exposed to a fall hazard of 126 inches. b) On or about 6/17/2024, at 118 Cayuga Street, Syracuse NY, 13204: A sub-contractor employee was installing roofing material on the low sloped roof above the window, estimated to be a 4:12 pitch, without fall protection and exposed to a fall hazard in excess of 126 inches. SOS Roofing was previously cited for an equivalent violation of the safety and health standard 1926.501(b)(13), which was contained in OSHA inspection 1659643, citation number 1, item number 2 and was affirmed as final order on 3/29/2024, with respect to a workplace located at 817 Second Street, Liverpool NY 13088.
