Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) On or about 6/17/2024, at 118 Cayuga Street, Syracuse NY, 13204: A sub-contractor employee was installing roofing material on the low sloped porch roof, estimated to be a 2:12 pitch, without fall protection and exposed to a fall hazard of 126 inches.  b) On or about 6/17/2024, at 118 Cayuga Street, Syracuse NY, 13204: A sub-contractor employee was installing roofing material on the low sloped roof above the window, estimated to be a 4:12 pitch, without fall protection and exposed to a fall hazard in excess of 126 inches.  SOS Roofing was previously cited for an equivalent violation of the safety and health standard 1926.501(b)(13), which was contained in OSHA inspection 1659643, citation number 1, item number 2 and was affirmed as final order on 3/29/2024, with respect to a workplace located at 817 Second Street, Liverpool NY 13088.
