29 CFR  1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:   a)  On June 17, 2025, and times prior thereto, an employee operating a forklift while wearing tennis shoes was exposed to crushing injuries due to the hazard of objects falling on feet or equipment rolling over feet.  b) Express Lube: On June 17, 2025, and times prior thereto, an employee washing vehicles, while wearing tennis shoes was exposed to crushing injuries due to the hazard of objects falling on feet or equipment rolling over feet.  c) Express Lube: On June 17, 2025, and times prior thereto, an employee performing oil changes while wearing soft toed boots was exposed to crushing injuries due to the hazard of objects falling on feet or equipment rolling over feet.  NOTE: Abatement certification is required for this item.
