29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about June 17, 2025, at the jobsite on building 4: employees were exposed to a 30'8" fall hazard while installing decking on a 5:12 pitched roof, in that, fall protection was not used.

29 CFR  1926.602(d):The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):  a) On or about June 17, 2025 at the jobsite building 4: employees were exposed to struck by and crushing hazards, while operating a JCB forklift to deliver decking materials to the roof, in that, the employer did not evaluate each powered industrial truck operator's performance at least once every three years.
