29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about June 17, 2025 at 1441 Villa St, Racine, WI, employees repairing a flat roof on a residential home with a 2:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 7 feet 2 inches from the roof eve to the ground below on the back of the home. (b) On or about June 17, 2025 at 1441 Villa St, Racine, WI, employees repairing a garage roof on a residential home with a 4:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 8 feet 3 inches from the eave of the roof to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about June 17, 2025 at 1441 Villa St, Racine, WI, the employer did not ensure the extension ladder used by employees to access and egress the residential roof was extended at least three feet above the upper landing. Exposing employees to a fall hazard of approximately 8 feet to the ground below.
